( GDPR-General Data Protection Regulation)


Senna Boumans - Benmasoud / Cascade Coaching; Practice for Integral Natural Medicine, is responsible for the processing of personal data as reflected in this privacy statement. Processing of personal data is done carefully in accordance with the requirements of the General Data Protection Regulation (AVG) as of May 25, 2018.
For proper treatment it is necessary for me as your treating therapist to create a file. This is also a legal obligation imposed by the Medical Treatment Agreement Act (WGBO/Wkkgz). Your file contains notes on your state of health and data on the examinations and treatments carried out.
Also included in the file are data that are necessary for your treatment and that I, after your explicit consent, have requested from another healthcare provider, for example the family doctor.


I do my best to ensure your privacy. This means, among other things, that I:

  • Handle your personal and medical information with care,
  • Ensure that unauthorized persons do not have access to your data.
  • As your treating therapist, I am the only one who has access to the data in your file. I have a legal duty of confidentiality (medical confidentiality).
  • The data in your file may also be used for the following purposes:
  • To inform other healthcare providers, for example when the therapy is completed or in case of a referral to another practitioner. This is done only with your explicit (written) permission.
  • For the use for observation, during my absence.
  • For anonymous use during peer review.
  • A small part of the data from your file is used for financial
    records so that I, or my administrator can prepare an invoice.
    If I want to use your data for any other reason, I will inform you first and ask your explicit permission.
    You have the right to inspect your client file at any time.
    You have the right to have your data deleted at any time.
    The data in your client file will, as required by law (WGBO), be kept for 20 years. Financial records will be kept for a minimum of 7 years.


PRIVACY ON THE CARE INVOICE


On the care bill you receive you will find the data requested by the health insurance company, so that you can claim this bill from your health insurance company.

  • Your name, address and place of residence.
  • Your client number and/or date of birth.
  • Insurance and/or policy number.
  • The date of the treatment.
  • A brief description of the treatment.
  • Performance code.
  • The cost of the consultation.

COOPERATION WITH OTHER ORGANIZATIONS


From the practice, I work with the following organizations regarding the processing of your data and have entered into a processing agreement with them:

  • Google.
  • Facebook.
  • Baat Accountants and Advisors.
  • One.Com Web Administrator.


COOKIE POLICY


What are cookies?
The website of Cascade Coaching; Practice for Integrative Natural Medicine uses cookies. A cookie is a simple small file sent with pages of this website and stored by your browser on your computer's hard drive. The information stored therein can be sent back to our servers during your next visit.

Use of persistent cookies
Using a permanent cookie, you will be recognizable when you visit the website again. The website can therefore be set specifically to your preferences. Even if you have given permission for the placing of cookies, we can remember this by means of a cookie. This avoids you having to repeat your preferences each time you visit, thus saving you time and making our website more user-friendly. Through this website a cookie is placed by the U.S. company Google, as part of the "Analytics" service. I use this service to track and get reports on how visitors use the website. Google may provide this information to third parties if Google is legally required to do so, or to the extent that third parties process the information on Google's behalf. I have no influence on this. I have not allowed Google to use the obtained analytics information for other Google services. The information Google collects is anonymized as much as possible. Your IP address is expressly not provided. The information is transferred to and stored by Google on servers in the United States. Google adheres to the Safe Harbor principles and is affiliated with the Safe Harbor program of the U.S. Department of Commerce. This means that there is an appropriate level of protection for the processing of any personal data.
